Some of the unique features of my plan include: $2,500 first day hospital coverage then $1,000 thereafter, 100% of Medicare/ RBRVS surgical benefit schedule with an anesthesia benefit of 25% the surgeons Benefit, $250 max benefit for diagnostic coverage that can be used in or out of hospital, limited Dr office visits, 24 hour Nurse line, diabetes care discount program, both Rx retail discount card and a mail order account. Guaranteed Issue* – Health Solutions Diamond Plan for $248 per month
If you are unable to qualify for the Health Choice Premier Plan, this might be the right plan for you. Some of the benefits of this plan include a $1,000 a day hospitalization benefit, 100% of Medicare RBRVS surgical benefit with anesthesia benefit of 20% the surgeons benefit, limited doctor office visits, diagnostic lab testing and x-rays, Rx discount retail card, $5,000 accident rider and $5,000 life insurance benefit.
*Applicants age 55 and older must be working at least 15 hrs per week to qualify. People receiving workers compensation, disabled, receiving disability benefits or Medicare eligible are not eligible to apply. (See brochure for specific benefits and limitations)
